Geriatrics Nurse Practitioner

Job Description:

Performs assessments in collaboration with the physician and as needed.
Develops and implements appropriate plans of care to PO participants.
Evaluates participant physical complaints and provides appropriate treatment.
Provides participants and caregiver teaching and education.
Functions as a member of the Interdisciplinary Team (IDT).
Provides daily oversight of PACE Organizations Clinic.
Perform rounds in the POs contracted nursing homes. Certification Requirements
- AANP
- ANCC
- BLS
- Controlled Substance (CSR/CDS)
- DEA State License Requirements
  - Indiana Additional License Requirements
    - 5-7 years of NP experience preferably in long-term care, Minimum 1 year working with the frail and/or elderly population
