Job Description:
A company is looking for a Licensed Vocational Nurse or a
Licensed Practical Nurse to join their Primary Care team.
Key Responsibilities:
Manage patients' healthcare needs through various communication
methods
Assist clinicians with administrative tasks and patient care
coordination
Identify barriers to optimal member health and implement strategies
to reduce risks
Required Qualifications:
Graduate of an accredited School of Practical/Vocational Nursing
(LPN, LVN)
Active, unrestricted LVN/LPN license in specified states
Experience in virtual-based practice
Spanish language fluency
3+ years of in-person outpatient or ambulatory LVN/LPN
experience
